Quick answer: Cube 1 magic charm + 3 perfect gems (any) to reroll that charm. The recipe keeps the charm size and ilvl, so a Hell Baal grand charm can still roll skillers and high life after dozens of tries. Farm high-level monsters for the base, then reroll toward skillers / Vita on grand charms, or MF / poison / Fine AR-damage on small charms. Sunder charms and other uniques are not this recipe.

The 3 perfect gem charm reroll recipe

In the Horadric Cube, combine:

Transmute. You get the same charm size back with a fresh prefix/suffix roll. Critically, the game keeps the item level — that is why high-ilvl junk grands are worth saving. You can reroll the same charm as many times as you have gems. There is no soft cap — only opportunity cost. Unique charms (Sunders, Gheed's, Annihilus, Torch) never use this formula.

Target affixes and ilvl thresholds

Affixes only appear when the charm's ilvl can reach their affix level (alvl). On drops, ilvl equals the monster level that dropped the charm — see magic affixes. Terror Zones raise monster level with your character, which is why late Hell TZ farming unlocks the top Vita rolls.

Numbers below are affix levels from charm tables (PureDiablo). Treat them as the minimum monster-level band you should farm for bases worth cube work:

A low-ilvl Normal grand charm will never become a skiller no matter how many perfect gems you burn. Ilvl first, gems second.

Grand charms: skillers, life and damage

Skillers (the main reroll target)

Only grand charms can roll +1 to a skill tree (Fletcher's, Sparking, Lion Branded, Fungal, Entrapping, and the rest of the classic prefix list — all alvl 50). That single mod is why GC rerolling dominates ladder economies. Plain skillers trade; skillers with life suffixes trade much harder.

Save every magic grand charm that drops from monsters high enough to unlock skillers. Identify junk trees you do not use, then cube them toward the trees your build (or the trade market) wants. Cross-check which trees are stash-worthy on items worth keeping.

Life (of Vita) on grand charms

Life suffixes on GCs scale hard: mid Vita bands start at alvl 77, and the famous +41–45 life band sits at alvl 91. Pairing a skiller prefix with high Vita is the jackpot. If your base came from a mid Hell trash mob, you can still hit skillers — but you will not hit top Vita until the ilvl is high enough. Farm Diablo, Baal, Nihlathak, Worldstone, or high Terror Zones for those bases.

Damage grands

Max-damage prefixes on grands (Fine / Sharp bands, plus Craftsmanship / Quality / Maiming suffixes) matter for some melee and bow setups, but most players prioritize skillers first. Reroll damage GCs only after your inventory skillers are stable, or when you already have a high-ilvl grand with a damage roll worth polishing.

Small charms: MF, poison and Fine AR/damage

Magic Find SCs

Small charms are the best MF per inventory square. The top magic suffix is of Good Luck (6–7% MF, alvl 33). Stack several for MF runs — see Magic Find and budget MF gear. Nightmare Act 4/5 and early Hell already produce ilvl high enough; you do not need Baal-level SCs just for MF.

Poison (“poison king”) SCs

The chase combo is a high poison prefix with a strong poison suffix (Pestilent + of Anthrax on small charms). Top poison prefixes sit at very high alvl (Pestilent SC is alvl 88), so treat these like Vita skillers: only reroll Hell endgame / TZ small charms. Lower poison rolls are common and usually not worth deep gem sinks.

Fine / 3 max / 20 AR

The Fine small-charm prefix (alvl 21) rolls +1–3 to maximum damage and +10–20 attack rating. Players chase the top of that range (often called 3/20). Because the alvl is low, almost any Hell SC can hit it — the bottleneck is RNG and gem cost, not farming location.

Also keep an eye on single-resist and Shimmering (all resist) small charms for PvM soft caps; those are often “keep if good, do not endlessly reroll” pieces.

Where to farm charms worth rerolling

When to reroll vs stop

Large charms can use the same recipe, but most ladder inventories skip them — they lose to two small charms or one skiller for the space they eat. Focus gems on grands and high-ilvl smalls unless you have a specific large-charm chase mod.
